Membership: $250/month
This flat monthly fee covers most of the care you’ll need at FIAT Primary Care Clinic — no insurance required, no copays, no claims, and no surprise bills tacked on after your visit. One predictable price gives Dr. Jazrawi the freedom to focus on you, not on billing codes.
Creighton Model Consultations: $50/session
Offered as a one-time service, each Creighton Model consultation is a 45-minute session for $50 — billed separately from your monthly membership, so you only pay for what you actually use.
